  • Messages sent through general website contact forms may not be encrypted to the same standard as our secure patient portal. Use the secure portal or call our office for protected health information or sensitive communications.

Appointments, Intake, and Billing

  • Online scheduling is subject to availability. Confirmations and cancellations are handled through the portal or phone.

  • Cancel or reschedule appointments according to the cancellation policy provided at booking. Late cancellations or no-shows may incur fees.

  • Providing accurate billing information is your responsibility, as is updating payment information.

Scope of Information
